Registration Form and Confidentiality Agreement

Individuals wishing to use the Financial Web Reports must register their request for access with the Director or Business Manager of their department/school and the appropriate Management Center Budget Office. Please complete the following form. You will be contacted with your ID and password within five working days of receiving your request.

Confidentiality Agreement

I understand that financial information from any source and in any form is available to me solely for the performance of my official duties as a Duke employee. I agree to protect the privacy and confidentiality of all financial information for which I have access and use it solely for the performance of my official duties. I also understand that through the Financial Services Web Reporting application I will have the ability to access financial information outside of my unit of responsibility. I agree NOT to access financial information outside my unit of responsibility, without documented authorization from the person with financial responsibility for that unit. I will keep my user ID and password secret and not share it with anyone. I am responsible for my use or misuse of confidential information.
