FEES FOR W-2'S:

For W-2 forms prior to 2001, requests should be submitted directly to the IRS.

An employee is required to complete a Tax Document Request Form to request a replacement of a lost or misplaced W-2, 1099, or 1042-S form.

The following requirements for Duke University must be met for requests:

No faxes or copies.
Employee Social Security number must appear.
Employee's name used during employment at Duke University .
Signature authorizing release of information.
In the event overnight postal service is required, the employee will pre-pay FedEx, UPS or the preferred vendor.
