SUBJECT: New Payroll Leave of Absence Form

Effective September 1, 2009, Corporate Payroll Services will introduce a new Payroll Leave of Absence Form. The new form requires departments to provide a begin and end date for most types of leaves. This simple change will improve payroll and department efficiencies by minimizing the number of paper forms departments send to Corporate Payroll Services and allowing iForms to be used for Transfers, Rate and Schedule Changes, and Cost Distribution Changes involving employees on a leave of absence.
